The Medical Verification of Eligibility Form (PDF) is an optional form that can be completed and included with your application to assist in the determination of your eligibility to utilize handyDART service.
handyPASS
A handyPASS allows your attendant to ride free on fixed route buses. It also allows you to participate in the taxi saver program.
handyPASS is a picture identification card. When applying for your handyPASS, please be sure to include two passport size photos with your handyDART application. Check the Yellow Pages of your telephone directory under ‘passport photos.’
