This page template is used for the /contact-us pages on all the subsites
A form on each subsite will need to be created in Contact Form 7 and have the respective shortcode added to the page content.
Eg.
Error: Contact form not found.
This page template also pulls in the settings from Appearance > Customize > Transit System Settings > Transit Service Info into the inner sidebar. This is used for contact info.
