  To apply for handyDART service and a handyPASS card, fill out this application form.

  These Guidelines will help you understand the criteria for handyDART service before you fill out a handyDART application form.

  A doctor or medical specialist may be asked for additional information to confirmation eligibility for handyDART.

  The caregiver confirms that the handyDART user is able to be left alone at the completion of a handyDART trip.
